Job summary

Interim HealthCare is seeking Registered Nurse Case Managers to provide in-home patient visits, assessments, and care coordination. You will develop and evaluate personalized care plans and work with a care team to ensure high-quality, compassionate home health services.

You will educate patients and families, monitor changes in health status, and help prevent hospital readmissions while supporting independence at home. A strong RN license and relevant home health experience are required.

Qualifications

• Active RN license: Current, unrestricted RN license in the state where you'll be practicing.
• Current certifications: CPR, AED, BLS, and First Aid as required for the position.
• Reliable transportation to travel to patient homes and other work locations.
• Experience: At least one year of professional RN experience in home health or a similar setting.
• Previous case management or utilization review experience is preferred.
• OASIS experience is preferred.
• Tracheostomy and/or ventilator experience is a plus, but not required.

Responsibilities

• Conduct in-person patient visits, interviews, and comprehensive assessments.
• Develop, implement, and continually evaluate individualized plans of care.
• Coordinate and oversee care provided by home health aides, LPNs, RNs, and other members of the care team.
• Communicate changes in patient conditions and collaborate with physicians and other healthcare providers to adjust care as needed.
• Educate patients and families on home health care, wound care, IV administration, medication management, and other aspects of their treatment.
• Promote preventative care, support continuity of care, and help reduce avoidable hospital readmissions.
