Job summary

RN.com in California is seeking an experienced RN Case Manager for a 12-week assignment to coordinate care and optimize resource utilization across acute care settings. You will perform comprehensive assessments and develop and evaluate care plans in collaboration with the care team.

The role requires an active CA RN license and AHA BLS, with 2+ years of case management experience, preferably in LTAC or STACH settings, and a Monday–Friday schedule.

Qualifications

• CA RN license and AHA BLS certifications are required.
• 2+ years RN Case Management experience, preferably in LTAC or STACH with discharge planning across acute care.
• Strong collaboration within an interdisciplinary team (IDT) and knowledge of utilization management.

Responsibilities

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Coordinates management of care and ensures optimum utilization of resources, service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration of functions of case management, utilization review and management and discharge planning. Provides ongoing support and expertise through comprehensive assessment, care coordination, plan implementation and overall evaluation of individual patient needs while ensuring patient preferences. Serves as a patient advocate through resource utilization, discharge planning and addressing the holistic needs of the patient.

MINIMUM REQUIRED QUALIFICATIONS –

• CA RN license
• AHA BLS
• 2+ years recent RN Case Management experience, with experience in a LTAC or STACH setting, MCG, discharge planning across acute care settings, strong IDT collaboration

LENGTH OF ASSIGNMENT – 12 weeks

SHIFT / HOURS PER WEEK – Monday - Friday 8a-4:30p/5p

START DATE – 10/12/2026
