Job summary

RN.com in California is seeking an experienced RN Case Manager for a 12-week assignment to coordinate care and optimize resource utilization across acute care settings. You will perform comprehensive assessments and develop and evaluate care plans in collaboration with the care team.

The role requires an active CA RN license and AHA BLS, with 2+ years of case management experience, preferably in LTAC or STACH settings, and a Monday–Friday schedule.

Qualifications

• CA RN license and AHA BLS certifications are required.
• 2+ years RN Case Management experience, preferably in LTAC or STACH with discharge planning across acute care.
• Strong collaboration within an interdisciplinary team (IDT) and knowledge of utilization management.

Responsibilities

• Coordinate care management and optimize resource use in compliance with external review agencies.
• Provide ongoing assessment, care planning, implementation and evaluation of patient needs.
• Advocate for patients through discharge planning, resource utilization and holistic care planning.
