Job summary

Bristol Hospice is seeking an RN Case Manager who provides compassionate, whole-person care and advocates for patients and families through their hospice journey. You will develop and implement individualized care plans, monitor vital signs, administer treatments with empathy, and educate families on symptom management.

Qualifications

  • Active RN license in good standing in the state you will work in.
  • CPR certification (current)
  • Valid driver’s license and reliable transportation
  • Strong observational and written/verbal communication skills
  • Flexible, team-oriented attitude and willingness to support additional duties as needed

Responsibilities

  • Develop and implement individualized care plans based on current and prior medical history.
  • Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
  • Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
  • Monitor and document vital signs, administer physician-ordered medications and treatments with skill and empathy.
  • Educate patients and families on care techniques and symptom management.
  • Track and communicate changes in patient condition, ensuring timely and effective responses.
  • Keep detailed clinical notes reflecting patient progress and quality of care.
  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
