Bristol Hospice - Rogue Valley Llc is seeking an RN Case Manager in Oregon to provide compassionate, whole-person care to patients and families. You will develop care plans, monitor vitals, administer treatments, educate families, and collaborate with interdisciplinary teams to ensure excellent hospice care.

This role requires an active RN license and CPR certification, with strong communication skills and reliable transportation.

Qualifications

• Valid RN license in the state you will work
• CPR certification current
• Strong observational and communication skills—written and verbal
• Willingness to support additional duties as needed

Responsibilities

• Make a true difference—support patients and families with compassionate care
• Develop and implement individualized care plans based on medical history
• Conduct assessments of physical, emotional, and spiritual needs throughout the care journey
• Monitor vital signs and document readings accurately and promptly
• Administer physician-ordered medications and treatments with empathy
• Educate patients and families on care techniques and symptom management
• Track changes in patient condition and respond with timely actions
• Keep detailed clinical notes reflecting patient progress and quality of care
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
