Job summary

Moments Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care on the road. This role integrates with company owners and supports entrepreneurial growth and clinical advancement.

Responsibilities include initial assessments, care plan refinement, direct nursing care, patient education, and coordinating care with a multidisciplinary team. Requires RN license and reliable vehicle; benefits include health coverage and a company car.

Qualifications

• Must be licensed as a Registered Nurse (RN) in the state of practice.
• Proficiency with Electronic Medical Records (EMR) software.
• Strong planning, organizing, and communication skills.
• Ability to solve problems autonomously and meet responsibilities with high standards.
• Valid Driver's License and reliable vehicle with current insurance.

• Maintain open communication with patients, families, and the healthcare team.
