Job summary

Moments Hospice in the United States is seeking an experienced RN Case Manager to assess patients, refine care plans, and coordinate nursing care. You will work closely with families and the healthcare team to ensure comfort and quality of life for patients at the end of life. Qualifications

• Licensed RN in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
