Job summary

Moments Hospice is seeking an RN Case Manager to provide compassionate in-home and on-the-road care, coordinating patient plans and educating families. You will work directly with the owners, support clinical advancement, and uphold high standards while managing a reasonable caseload.

This role includes initial assessments, hands-on nursing, clear communication with patients and teams, and participation in interdisciplinary meetings.

Qualifications

• Must be licensed as RN in the state of practice.
• Proficiency with EMR systems.
• Excellent planning, organizing, and communication.
• Ability to solve problems autonomously while maintaining high standards.
• Valid driver''s license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Responsibilities:

• Valid Driver's License and reliable vehicle with current insurance
