Job summary

AMN Healthcare seeks an experienced RN Case Manager for an assignment in Lebanon, NH. You will manage a daily inpatient caseload, participate in interdisciplinary rounds, and coordinate post-discharge plans with patients and families.

The role requires a BSN, CCM certification, and 3 years inpatient experience, with 1+ year in care management. Epic EMR is used for documentation; 13-week assignment with weekday hours.

Qualifications

POSITION SUMMARY – RN Case Manager

POSITION DUTIES

Oversee a daily caseload of patients on the inpatient units with a variety of care needs. Will attend interdisciplinary rounds. Complete Initial assessments. Facilitates the development of a plan of care in collaboration with the patient, family, caregiver and health care team. Communicate consistently with medical teams and patient regarding progress of care needs related to post discharge care needs, such as: home health, inpatient rehab. Will interact with post acute care referral sources to establish a plan. Document and Place orders for home health, DME, nutritional needs within EPIC EMR.

Minimum Required Qualifications –

• NH/compact RN license
• BSN
• Certified Case Manager (CCM)
• 3 years inpatient clinical experience
• 1+ year Care Management experience

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

8-4:40 M-F. No weekends or on call.
