POSITION SUMMARY – RN Home Health Hospice Case Manager

Screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the home health hospice patient. Will cover various towns in VT and NH.

Minimum Required Qualifications

• Compact or VT & NH unencumbered RN license
• Graduate of an accredited school of nursing
• BLS
• 2 years of experience in Home Health and Hospice

LENGTH OF ASSIGNMENT

16 weeks

SHIFT / HOURS PER WEEK

8:00am-4:30pm, Mon-Fri with rotating weekends
