Qualifications

• NH/compact RN license required.
• BSN required.
• Certified Case Manager (CCM) certification required.
• 3 years inpatient clinical experience required.
• 1+ year care management experience required.

Responsibilities

• Oversee a daily caseload of patients on inpatient units.
• Attend interdisciplinary rounds and develop a plan of care with patient, family, caregiver and health care team.
