Job summary

AMN Healthcare is seeking an experienced RN Case Manager for hospice care. You will assess patients' physical, emotional, and psychosocial needs, coordinate individualized end-of-life care plans, manage symptoms and pain, and educate patients and families within a home setting.

Join our team for an 13-week assignment with typical 8a-4:30p hours, mostly weekdays and rotating weekends. Epic experience preferred; pay range is $1800–$2500 per week; start date ASAP.

Qualifications

• Active RN license in ME or compact state.
• BLS certification.
• 2 years of recent home health/hospice case management experience.

Responsibilities

• Assesses patients' physical, emotional, and psychosocial needs; develops end-of-life care plans; manages symptoms and pain.
• Educates patients and families; collaborates with interdisciplinary hospice team; monitors care outcomes.

StartDate: 11/23/2026 Pay Rate: $1800.00 - $2500.00

POSITION SUMMARY – RN Case Manager - Hospice

POSITION DUTIES – Assesses patients' physical, emotional, and psychosocial needs; develops and coordinates individualized end-of-life care plans; manages symptoms and pain control; educates patients and families; collaborates with interdisciplinary hospice team members; monitors care outcomes; and provides ongoing support to ensure comfort, dignity, and quality of life in the home setting. Productivity is 25 visits per week, looking for experienced with admissions.

Minimum Required Qualifications –

• ME/compact RN license
• BLS
• 2 years recent home health/hospice case management experience

PREFERRED QUALIFICATIONS

Skilled experience with PICC lines/dressing changes, trachs, port access, ostomies, chest tubes, Pleurex drains, S/p catheters, lab draws/UA/C&S, etc.

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

8a-4:30p, Mostly M-F with rotating weekend every third weekend
