Job summary

AMN Healthcare seeks an RN Case Manager to coordinate patient care on Cape Cod. You will work with an interdisciplinary team to plan and implement an individualized program of care, assess needs using Care Maps, and coordinate with third parties to meet care goals and reimbursement.

Requirements include MA RN license, 3 years acute care in the past 5 years, knowledge of InterQual, and experience in discharge planning. 18–26 week assignment with 40 hours/week; Epic experience is preferred.

Qualifications

• MA RN license required.
• 3 years acute care experience in past 5 years in hospital or case management.
• Working knowledge of InterQual or equivalent.
• Recent knowledge in discharge planning and utilization review.

Responsibilities

• Coordinate and facilitate patient care for a defined population.
• Collaborate with interdisciplinary team to plan and implement individualized care.

POSITION SUMMARY - RN Case Manager

POSITION DUTIES

Responsible for the timely facilitation and coordination of patient care for specific patient population or area of assignment. Work effectively with the interdisciplinary team so as to plan and implement an individualized program of care that promotes high quality, efficient and cost-effective care. Assess patient needs utilizing established standards/Care Maps and other approved clinical guidelines. Through ongoing communication and coordination with third party representatives, facilitate care that will meet established patient care needs while optimizing reimbursement.

Minimum Required Qualifications -

• MA RN license
• 3 years acute care experience within the past 5 years with broad clinical experience in a hospital setting or case management.
• Working knowledge of InterQual, or equivalent system
• Demonstrate recent knowledge/experience within past 4 years in Discharge Planning and Utilization review.

Preferred Qualifications -

• BSN
• CCM
