AMN Healthcare is seeking an experienced RN Case Manager in Cape Cod, MA to coordinate patient care with the interdisciplinary team, assess needs using Care Maps and clinical guidelines, and promote high-quality, cost-effective care.
The role requires MA RN license, 3 years acute care/case management, and knowledge of InterQual; the assignment is 40 hours per week with a 13-week duration, using Epic, with potential for growth.

– Responsible for the timely facilitation and coordination of patient care for specific patient population or area of assignment. Work effectively with the interdisciplinary team so as to plan and implement an individualized program of care that promotes high quality, efficient and cost-effective care. Assess patient needs utilizing established standards/Care Maps and other approved clinical guidelines. Through ongoing communication and coordination with third party representatives, facilitate care that will meet established patient care needs while optimizing reimbursement.
