Heart to Heart Hospice is seeking an RN Case Manager to provide comprehensive assessments and develop individualized care plans for patients with life-limiting illnesses. The role involves coordinating care with families, physicians, and IDG to ensure timely, compliant service.

The position requires a current RN license, nursing degree, and reliable transportation. On-call duties and benefits are included in a supportive team environment across our service areas.

Formación

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License is required.

Responsabilidades

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and recommend revisions to the plan as necessary.
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:
