Job summary

Heart to Heart Hospice is seeking an RN Case Manager to lead comprehensive in-home and community-based care for patients with life-limiting illnesses in Michigan. You will assess needs, coordinate a multidisciplinary plan, and ensure timely documentation and compliance with regulations, while supporting patients and families.

Qualifications

• Current State RN License required.
• Two years of RN experience preferred in healthcare/clinical setting.
• Ongoing valid auto liability insurance and valid driver's license.

Responsibilities

• Conduct initial and ongoing assessments of patient needs including physical, psychosocial, environmental and ADLs risks.
• Implement and revise the individualized Plan of Care (POC) as needed.
• Coordinate care with patient, family, physicians and healthcare team.
• Attend IDG meetings and participate in care planning and prioritization.
• Provide on-call services to patients and families as assigned.
• Submit documentation in a timely manner.
• Perform duty as required and ensure compliance with laws and policies.
• Maintain compliance with Medicare regulations and agency policies.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
