Job summary

RN.com is seeking an experienced RN Case Manager in Tennessee to coordinate patient care from admission through discharge, ensuring treatments align with each patient’s medical needs and recovery goals.

You will collaborate with physicians, nurses, and external providers to remove barriers to care and support safe, efficient discharge planning. The assignment runs for 13 weeks with a shift from 8:00AM to 4:30PM and rotating weekends.

Qualifications

• Active RN license (TN/compact)
• BLS certification required.
• At least 2 years of case management experience in an acute hospital setting.
• Acute hospital experience within the last year, with no more than a 90-day break.

MINIMUM REQUIRED QUALIFICATIONS

LENGTH OF ASSIGNMENT

SHIFT / HOURS PER WEEK
