Job summary

AMN Healthcare is seeking an RN Case Manager in Grand Junction, CO for an 8-week assignment. You will drive the Case Management Plan, coordinate with an interdisciplinary team, and ensure high-quality patient care while complying with relevant billing regulations.

The role requires an unrestricted CO license, BLS, and a BSN, with 3+ years hospital experience and 5+ years in case management. Start ASAP; full-time during assignment.

Qualifications

• 3+ years of clinical experience in a hospital setting required.

Position Summary – RN Case Manager

Position Duties

Responsible for the ongoing development, implementation, and evaluation of processes of the Case Management Plan. The Case Management function is a highly collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client’s/patient’s health and human service’s needs. Provides advocacy, communication, collaboration, resource management, and education to interdisciplinary team members. Effective allocation of the hospital’s resources while encouraging provision of high-quality patient care is essential in this role. Is cognizant of regulations and policies of all review organizations relating to billing of the fiscal intermediaries and implements processes and changes to meet the Federal/State regulations.

Minimum Required Qualifications

• Unrestricted CO/compact RN license
• BLS
• Bachelor’s Degree in Nursing
• 3+ years of clinical experience in a hospital setting
• 5+ years of case management experience in a hospital setting

Preferred Qualifications

• Certification in case management or willingness to obtain within 2 years of employment
• Master’s degree

Shift / Hours per Week

Full-time. Will discuss shift in interview.
