Elara Caring is seeking a Home Health RN Case Manager to serve patients in the Northampton-Easthampton territory. You will perform skilled nursing visits, assess patients, and develop care plans in patients' homes, coordinating with physicians.

This full-time role offers a guaranteed $87,000 to $97,000 salary plus a productivity bonus, mileage reimbursement, flexible scheduling, and comprehensive benefits, including health coverage and a 401(k).

Qualifications

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to work independently and travel throughout assigned territory.
• Valid driver’s license, auto insurance, and reliable transportation.
• Ability to sit, stand, bend, lift and move intermittently and lift 50–100 pounds.

Responsibilities

• Guide patients through home health care from assessment to discharge with skilled nursing visits.

What You’ll Do

• You’ll guide patients through their home health care, from assessment to discharge. That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians. You’ll also teach patients and caregivers how to manage medications, treatments, and health conditions at home.
