Job summary

Elara Caring is seeking a Home Health RN Case Manager in the Northampton area to deliver compassionate, patient-centered care in patients’ homes. You will guide assessment, plan development, and discharge processes while coordinating with an interdisciplinary team.

This role emphasizes independent decision-making, traveling across the Northampton/Easthampton territory, and building strong relationships with patients and families.

Qualifications

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to work independently and travel throughout assigned territory.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds.

Responsibilities

• Guide patients through home health care from assessment to discharge.
• Provide skilled nursing visits and update care plans.
• Manage an assigned caseload and coordinate with physicians and clinicians.
• Teach patients and caregivers how to manage medications, treatments, and health conditions at home.

What You’ll Do

You’ll guide patients through their home health care, from assessment to discharge.
