Qualifications

• RN license with active state license.
• At least 1 year of experience in home health or case management.
• Strong critical thinking and communication abilities.
• Comfort using EHR systems (Kantime, Axxess, HCHB, WellSky preferred).
• Reliable transportation and a valid driver’s license.

Responsibilities

• Manage the Care Journey: serve as the primary contact for a caseload of home health patients, coordinating services.
• Conduct Comprehensive Assessments: complete OASIS assessments and develop care plans with the interdisciplinary team.
• Coordinate Services: work with physicians, nurses, therapists, social workers, and home health aides for timely care.
• Monitor and Evaluate: track patient progress and update care plans; communicate with families/caregivers.
• Documentation: maintain accurate and timely clinical documentation per Medicare guidelines and agency policy.
• Advocate for the Patient: provide education, resources, and emotional support to promote independence and wellness.

What We're Looking For

• Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
• Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
