POSITION SUMMARY

– RN Case Manager

POSITION DUTIES

– Coordinates and oversees patients' individualized care plans, collaborating with physicians, therapists, nurses, patients, and families to facilitate rehabilitation goals, manage resources, monitor progress, and ensure a safe and efficient transition to the next level of care.

MINIMUM REQUIRED QUALIFICATIONS

• CO/compact RN license
• BLS
• 1+ year recent case management experience in an acute hospital setting

LENGTH OF ASSIGNMENT

– 13 weeks

SHIFT / HOURS PER WEEK

– 8:00 AM-4:30 PM. Weekend Hours: 1-5.
