Job summary

Bristol Hospice in Des Plaines, IL seeks an experienced RN Case Manager to coordinate compassionate, patient-centered care for individuals and families at every stage of the hospice journey.

You will develop individualized care plans, monitor vital signs, administer medications, educate families, and collaborate with interdisciplinary teams to ensure comfort, dignity, and quality of care.

Qualifications

• Current RN license in good standing in Illinois.
• CPR certification and valid driver’s license.
• Strong communication skills with patients and families.
• Commitment to hospice philosophy and patient dignity.

Responsibilities

• Develop and implement individualized care plans based on patient history.
• Provide one‑on‑one support to patients and families, ensuring comfort and understanding.
• Monitor vital signs and document progress accurately.
• Administer physician‑ordered medications with empathy.
• Educate families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one‑on‑one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician‑ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up‑to‑date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Contribute to other supportive duties as part of a compassionate, mission‑driven team.
• Perform additional duties as assigned.

Qualifications

• A heart for hospice care and a commitment to exceptional service.
• A valid Registered Nurse license in good standing in the state where you will work.
• A valid driver’s license and reliable transportation.
• Current CPR certification.
• Strong observational and communication skills—both written and verbal.
• A flexible, team‑oriented attitude and willingness to support additional duties as needed.
