Job summary

Bristol Hospice in Colorado Springs, CO is seeking an experienced RN Case Manager who will advocate for patients and families, delivering compassionate, whole-person care.

Qualifications

• Active RN license in good standing in Colorado.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Strong observational and communication skills.
• Flexible, team-oriented attitude.

Responsibilities

• Develop and implement individualized care plans based on patient history.
• Deliver one-on-one support to patients and families with comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs accurately.
• Administer physician-ordered medications and treatments.
• Educate families on care techniques and symptom management.
• Track and communicate changes in patient condition.
• Keep up-to-date clinical notes reflecting patient progress.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Contribute to other supportive duties as part of a mission-driven team.
• Additional duties as assigned.

What You’ll Bring to the Team
