Job summary

Bristol Hospice in Oklahoma City is seeking a dedicated RN Case Manager to provide compassionate, whole-person care and advocate for patients and their families on this meaningful journey.

As an RN, you will develop individualized care plans, deliver one-on-one support, assess physical, emotional, and spiritual needs, monitor vitals, administer physician-ordered medications, educate families, and collaborate with interdisciplinary teams to ensure high-quality end-of-life care.

Qualifications

• A heart for hospice care and a commitment to exceptional service.
• A valid Registered Nurse license in good standing in the state you will work in.
• A valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills—both written and verbal.
• A flexible, team‑oriented attitude and willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician‑ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Contribute to other supportive duties as part of a compassionate, mission‑driven team.
• Additional duties as assigned
