Job summary

CareSphere Homecare – Pediatric Skilled Division in Bethlehem, PA is seeking a Part-Time Registered Nurse to serve as a Home Care Case Manager. The role is office-based with local travel and typically includes 3-4 brief home visits daily for assessments.

The position offers a Weekday schedule (Monday–Thursday, 2:00 PM–7:00 PM) with no weekends, no holidays, and no on-call. Active PA RN license and reliable transportation are required.

Qualifications

• Active PA RN license required.
• Valid driver’s license and reliable vehicle.
• Personal laptop.
• PPD or QuantiFERON test.
• CPR Certification.
• Minimum 1 year of nursing experience (home health preferred).

Responsibilities

• Schedule and perform Q60 day HHA Supervisory/Recertification visits.
• Schedule and perform Start of Care assessment visits.
• Ongoing Case Management for client and HHA oversight.
• Evaluate the client status and the skills of staff members.
• Complete timely and accurate clinical assessments within 3 days of the visit.
• Complete timely and accurate 485/ Plan of Care for patients within 3 days of the visit.
• Provide patient, family, or caregiver education.

About the Role:
