Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate RN Hospice Case Manager to join our team serving patients and families across Southern Palm Beach County. You will coordinate care and advocate for patients, helping them maintain comfort and dignity at the end of life.

You will collaborate with the Interdisciplinary Group, develop individualized plans of care, monitor patient conditions, and provide education to families.

Qualifications

• Registered Nurse licensed in Florida with case management experience.
• Strong crisis intervention and teaching skills for patient/family education.
• Knowledge of hospice nursing concepts and thanatology.

Responsibilities

• Serve as an advocate for patients and families, ensuring needs are addressed.
• Coordinate care under physician direction with the interdisciplinary team.
• Develop and implement individualized plans of care as part of the IDG.
• Monitor patient conditions, deliver interventions, and provide education to enhance comfort.
• Address concerns promptly to maintain satisfaction and trust.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and dedicated RN Hospice Case Manager to join our team serving patients and families across Southern Palm Beach County. In this role, you’ll have the unique opportunity to provide comfort, dignity, and support to individuals facing advanced illness—while working alongside a collaborative, mission-driven team. Your schedule would be: 8:30am - 5pm & every other weekend

What you’ll Do

As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey. You will:

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
