Job summary

Trustbridge Hospice, part of Empath Health, is seeking an RN Hospice Case Manager to coordinate care for patients and families across Southern Palm Beach County. The role emphasizes advocacy, clinical expertise, and education for a dignified, comfortable end-of-life journey.

The position involves collaboration with the interdisciplinary team, developing individualized plans of care, and ensuring timely communication to maintain high satisfaction and trust. Florida licensure and BLS are required.

Qualifications

• Registered nurse licensed in Florida.
• BLS certification required.
• Prior experience in hospice, home health, or public health preferred.

Responsibilities

• Coordinate comprehensive patient care under physician direction and with the IDG.
• Advocate for patients and families to ensure needs are addressed.
• Develop and implement individualized plans of care.
• Monitor patient conditions and provide interventions and education to improve comfort and quality of life.
• Address family questions promptly to build trust and satisfaction.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and dedicated RN Hospice Case Manager to join our team serving patients and families across Southern Palm Beach County. In this role, you’ll have the unique opportunity to provide comfort, dignity, and support to individuals facing advanced illness—while working alongside a collaborative, mission-driven team. Your schedule would be: 8:30am - 5pm & every other weekend

What you’ll Do

As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey. You will:

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
