Qualifications

• Current unrestricted RN licensure in state of practice.
• 2+ years of RN experience.
• Willingness to travel up to 75% with valid driver's license.
• Able to work with diverse care teams in home settings.
• Computer literate and able to navigate the Internet.

Responsibilities

• Assess health status and establish goals for identified needs.
• Plan, implement, and evaluate plan of care with the team.
• Collaborate with multidisciplinary team to address barriers to care.
• Document in the electronic medical record accurately.
• Provide counseling and education to patients and families.
