Cambia Health Solutions is seeking a Remote Care Management Nurse to join our Clinical Services team. The role focuses on case management, disease management, and care coordination to deliver person-focused, cost-effective care for members while working from home in OR, WA, ID or UT.

The ideal candidate is a Registered Nurse with a current license, strong clinical experience, and the ability to participate in multidisciplinary collaboration.

Qualifications

• Nursing or health/human services degree (Associate's or Bachelor's minimum).
• Active, unrestricted RN licensure in the U.S. and eligible for licensure in ID, OR, UT and WA.
• Minimum 3 years direct clinical care or case management experience.

Responsibilities

• Conduct case management activities: assessment, planning, implementation, coordination, monitoring and evaluation.
• Apply clinical expertise to ensure medical policy compliance and evidence-based care.
• Collaborate with physicians, internal/external teams to resolve issues and drive quality improvement.
• Provide professional, confidential communications and timely documentation.
• Coordinate resources and manage assignments to meet timelines.
