Jobot is seeking an RN leader to oversee the Case Management department at a renowned Hudson Valley academic medical center, located a short drive from NYC. The role focuses on guiding RN case managers, coordinating care transitions, and ensuring high-quality, cost-effective patient care with strong collaboration among physicians, nurses, social workers, and post-acute partners.

Work schedule is typically Monday through Friday, 8:00 am to 4:30 pm.

Responsibilities

• Oversee the hospital’s Case Management department and lead RN case managers in coordinating high-quality, cost-effective patient care.
• Manage care transitions, utilization review, discharge planning, and collaboration with physicians, nurses, social workers, insurance providers, and post-acute care facilities.
• Ensure appropriate resource utilization, regulatory compliance, patient advocacy, and timely discharge planning while promoting optimal clinical and financial outcomes.
