Job summary

Jobot is actively seeking a Hospital based RN Case Manager for a full-time role in the Hudson Valley, NY. The position features a Monday–Friday schedule with rotating weekend coverage and comprehensive benefits.

Responsibilities include coordinating care from admission through discharge, developing individualized care plans, and applying clinical judgment to optimize outcomes while adhering to hospital policies and standards.

Qualifications

• Minimum 3-5 years of clinical medical-surgical, ICU or ER experience.
• Experience in managed care performing utilization review and discharge planning preferred.
• Current NYS RN license required.

Responsibilities

• Collaboratively implements and facilitates patient care plans from pre-hospital to post-discharge while managing an assigned unit caseload.
• Assess, plan and monitor options and services to provide an individualized plan for the patient across the continuum of care.
• Use independent judgment and discretion; continuum must be reassessed as an ongoing process for the patient.
• Utilize clinical knowledge, communication, problem solving and conflict resolution to effect change.
• Enforce hospital policies regarding appropriateness and timeliness of clinical care.
