Job summary

LeoForce is seeking a Hospital RN Case Manager in the Hudson Valley area for a full-time role. The RN Case Manager collaborates on patient care plans from pre-hospital to post-discharge and manages an assigned unit caseload.

The position requires 3–5 years of clinical experience in medical-surgical, ICU, or ER settings, with NYS RN licensure. CCM certification is preferred, and the schedule is weekdays with rotating weekend coverage.

Qualifications

• Minimum 3–5 years relevant clinical medical surgical, ICU or ER experience preferred.
• Experience in managed care performing utilization review and discharge planning preferred.
• CCM certification preferred.
• Current NYS RN license required.

Responsibilities

• Assess, plan, and monitor options and services to provide an individualized plan across the continuum of care.
• Collaborate on patient care plans from pre-hospital to post-discharge and manage assigned unit caseload.
• Use clinical judgment, communication, problem solving, and conflict resolution to support patient care without delay.
• Enforce hospital policies and procedures regarding appropriateness and timeliness of clinical care.
