Job summary

Bassett Healthcare Network in New York is seeking a Case Manager RN to support physicians and the interdisciplinary team, coordinating utilization management, discharge planning, and care facilitation to improve outcomes and patient satisfaction while managing costs.

You will lead care transitions, document in Milliman and EPIC, collaborate across disciplines, and ensure timely, high-quality care. Requires NYS RN license, case management certification, and 5+ years of clinical experience.

Qualifications

• 4 Year/Bachelor's Degree in Nursing or Social Work, preferred (of willingness to complete a Bachelor's Degree within a specified time period)
• 2 Year/Associates Degree in Human Services related field, required
• Minimum 5 years clinical experience in a healthcare setting, preferred
• Current NYS licensure as a Registered Professional Nurse, if applying as a nurse, required
• Case Management certification, preferred
• Case Management certification within 5 years from date of hire, required

Responsibilities

• Facilitates precertification/payor authorization processes
• Coordinates care across the continuum and discharge planning
• Completes utilization management and quality screening for assigned patients
• Documents in Milliman and EPIC
• Communicates with physicians and payors to ensure timely, appropriate care
