Job summary

Green Key Resources is assisting a leading hospital in Hudson Valley to recruit a senior Case Management leader (RN or LMSW) to guide utilization review, discharge planning, and care coordination. You will manage department budgets, supervise staff, ensure regulatory compliance, and act as a liaison to payers and medical teams to optimize patient flow and resource use.

5+ years of leadership experience in acute care case management and thorough knowledge of state and federal guidelines are

• Must be licensed RN or LMSW in NY.
• 5+ years leadership in Case Management in an Acute Care Setting.
• Understanding of assessment, planning, and managing patient care.

Background:
