Job summary

UMMC is seeking a Manager of Case Management - RN for Coordinated Care on its Main Campus Jackson. The role supervises daily tasks of a Case Management team, coordinating staffing, performance, scheduling, and data reporting to optimize resources and patient placement.

The ideal candidate will have four years of RN nursing experience in case management, utilization review or discharge planning, plus an RN license and CCM/ACM certifications.

Qualifications

• Four years nursing experience in case management or related field.
• One year of nursing leadership experience.
• Valid RN license. CCM and ACM certifications.
• Knowledge of utilization review, discharge planning, and managed care reimbursement.
• Strong interpersonal, verbal, and written communication skills.

Responsibilities

• Determines staffing needs and participates in hiring.
• Coordinates departmental activities by scheduling work assignments and priorities.
• Identifies key competencies and develops assessment processes from hiring through performance evaluations.
• Evaluates employee performance through review of assignments and outcomes.
• Provides daily supervision, including workload determination, training oversight, coaching, performance evaluation, and data collection.
• Provides coaching and mentorship to staff for professional development.
• Develops and communicates work instructions and assists staff with complex tasks.
• Documents findings and plans; tracks and analyzes data to evaluate benchmarks and performance metrics.
• Ensures efficient resource utilization by removing barriers to productivity.
• Serves as a resource for resolving complex patient placement issues.
• Supports ongoing staff development activities with department leadership.
• Provides backup coverage to ensure departmental goals and continuity.
