Job summary

UMMC is seeking an RN Case Manager for PRN hours at the Main Campus Jackson. The role coordinates comprehensive care for a defined patient group, leading a multidisciplinary team to ensure quality outcomes while controlling costs.

Requirements include 3 years of RN experience with at least 1 year in case management or related areas, and an active RN license (case management certification preferred). Strong communication and collaboration skills are essential.

Qualifications

• Three (3) years of RN experience, one (1) year in case management, utilization review, utilization management, or performance improvement.
• Valid RN license, RN case management certification preferred.
• Knowledge of patient care for a specified population and strong communication skills.
• Ability to work in a team, multi-task, and manage time and resources effectively.

Responsibilities

• Performs and documents a comprehensive assessment for a specified patient group to benefit from case management.
• Participates in the development of a care plan with patients, family, and the interdisciplinary team.
• Reassesses and documents clinical needs and case management interventions.
• Facilitates coordination and access to resources for the patient's needs.
• Coordinates and implements case management activities to meet goals.
• Leads case management activities within specific patient populations.
• Monitors progress and adjusts the plan as conditions change.
• Evaluates appropriateness and medical necessity of the plan of care.
• Collaborates with social workers and teammates to discuss services and support.
