Job summary

PAM Health Specialty Hospital of Stoughton is seeking an experienced Director of Case Management to lead the hospital-wide program and coordinate care across disciplines. You will supervise case managers, review patient records for appropriate utilization, and drive discharge planning to improve outcomes.

RN licensure in the state is required; other licensure types may be accepted per state regulations. BLS is required; BSN/MSN preferred; CM certification preferred.

Qualifications

• RN licensure in the state of employment required or preferred.
• Social workers may be accepted in lieu of RN licensure if licensed or hold a bachelor’s degree in social work as required by state regulations.
• LPN, MSW, LSW, PT, OT, SLP and RT licensure may be accepted in accordance with state regulations.
• Current BLS certification required; BSN/MSN preferred; CM program certification preferred.
• Three to five years of case management experience; prior management experience preferred; IRF or LTACH experience preferred.

Responsibilities

• Coordinate the hospital‑wide case management program.
• Oversee review of patient records to ensure proper utilization of services.
• Assist case managers with timely care coordination and discharge planning.
• Supervise daily operations of the case management department to improve financial and clinical outcomes.
• Engage in fiscal planning and regulatory compliance; promote quality of care through collaboration.
