Job summary

PAM Health Rehabilitation Hospital of Allen is seeking an experienced Director of Case Management. This role oversees the hospital-wide case management program, reviews patient records for appropriate service utilization, and supports discharge planning to ensure quality outcomes.

The position requires RN licensure and 3–5 years of case management experience, with prior management preferred. Eligible candidates will have BLS certification; BSN/MSN is preferred.

Qualifications

• RN licensure in the state where the hospital resides required.
• BSN/MSN preferred.
• Social workers may be accepted in lieu of RN licensure if they are licensed or hold a bachelor's degree in social work, per state regulations.
• Current BLS certification required.

Responsibilities

• Coordinate hospital-wide case management program.
• Review patient medical records to ensure proper utilization of hospital services.
• Assist case managers with timely care coordination and discharge planning.
