Job summary

ARMStaffing is seeking a Registered Nurse Case Manager in California for a 13-week contract, Monday–Friday, 8:00 AM–4:30 PM. The role coordinates care delivery, drives discharge planning, and ensures regulatory compliance with LTAC/STACH requirements.

With at least 2 years in RN case management, you will collaborate with interdisciplinary teams, advocate for patients, and promote cost-effective, high-quality care while maintaining positive outcomes.

Qualifications

  • Minimum 2 years of RN Case Management experience.
  • Experience in LTAC or STACH settings.
  • Familiarity with MCG and discharge planning.
  • Strong collaboration with interdisciplinary teams.
  • California RN license and BLS certification must be attached.

Responsibilities

Job Description

The Registered Nurse Case Manager coordinates management of care, ensuring optimum utilization of resources, service delivery, and compliance with external review agencies. The role provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. The Case Manager enhances the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness through the integration of functions of case management, utilization review, and discharge planning.

Responsibilities
  • Conduct comprehensive patient assessments to identify needs and preferences.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Advocate for patients by addressing their holistic needs and preferences during care processes.
  • Ensure compliance with healthcare regulations and standards.
  • Facilitate discharge planning and transitions of care to promote continuity and effective resource utilization.
Required Experience / Certifications / Licensure
  • Minimum of 2 years of recent RN Case Management experience.
  • Experience in Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
  • Proficiency in Medical Criteria Guidelines (MCG) and discharge planning across acute care settings.
  • Strong interdisciplinary team collaboration skills.
  • Current California RN license (must be attached to resume).
  • Basic Life Support (BLS) certification from the American Heart Association (must be attached to submission packet).
