Job summary

The ideal candidate has 2+ years of clinical experience, acute care exposure, and proficiency in EPIC with familiarity of MCG/InterQual criteria.

Qualifications

• Minimum of 2 years of clinical experience, with at least 1 year in an acute care setting.
• Experience in case management, utilization management, MCG criteria, InterQual criteria, and EPIC is required.
• Acute hospital management experience is highly preferred.

Responsibilities

• Coordinate integration of case management into patient care and discharge planning.
• Facilitate patient care progression throughout the continuum of care in a cost-effective manner.
• Monitor length of stay and address barriers to expedite discharge.

Shift: 5 Days, 8 Hours/Day. May be asked to arrive at 7:30 AM and must stay until all cases are finished for the day.

Job Description

The Care Manager (CM) II works in collaboration with patients/families, physicians, and multidisciplinary team members to ensure patient progression through the continuum of care and to develop a care plan for each assigned patient from admission through discharge. The CM is responsible for identifying, initiating, and managing optimal patient flow to enhance continuity of care and ensure smooth transitions, patient satisfaction, patient safety, and length of stay management.

Responsibilities

• Coordinate the integration of case management functions into the patient care and discharge planning processes.
• Facilitate patient care progression throughout the continuum of care in an efficient and cost-effective manner.
• Monitor patient length of stay and work with interdisciplinary team members to address barriers and expedite discharge.
• Implement and monitor the patient's plan of care to ensure effectiveness and appropriateness of services.
• Collaborate with medical and nursing staff to eliminate barriers to efficient care delivery.
• Conduct assessments to develop comprehensive discharge planning assessments and care plans.
• Act as a patient advocate regarding treatment decisions and resource coordination.
• Initiate discharge planning at the time of admission and make necessary referrals based on assessment information.
• Provide education and support related to patients' goals of care.
• Participate in multidisciplinary rounds and clinical performance improvement activities.

Required Experience

• Minimum of 2 years of clinical experience, with at least 1 year in an acute care setting preferred.
• Experience in case management, utilization management, MCG criteria, InterQual criteria, and EPIC is required.
• Acute hospital management experience is highly preferred.

Certifications/Licensure

• RN or LMSW licensure in the state of Texas is required for new hires. LBSW accepted for associates with 5+ years of demonstrated success in the Care Manager I role.
• Certification in Case Management is preferred.
• BLS certification is preferred.
