Job summary

ARMStaffing is seeking a Care Manager II in Longview, Texas for a 13-week contract. The role involves coordinating case management, discharge planning, and resource coordination from admission through discharge, working with patients, families, and the care team to optimize flow and outcomes.

The ideal candidate has 2+ years of clinical experience, acute care exposure, and proficiency in EPIC with familiarity of MCG/InterQual criteria.

Qualifications

• Minimum of 2 years of clinical experience, with at least 1 year in an acute care setting.
• Experience in case management, utilization management, MCG criteria, InterQual criteria, and EPIC is required.
• Acute hospital management experience is highly preferred.

Responsibilities

• Coordinate integration of case management into patient care and discharge planning.
• Facilitate patient care progression throughout the continuum of care in a cost-effective manner.
• Monitor length of stay and address barriers to expedite discharge.
• Implement and monitor the plan of care for effectiveness and appropriateness.
• Collaborate with medical and nursing staff to eliminate barriers to care.
• Conduct assessments to develop discharge planning and care plans.
• Act as a patient advocate regarding treatment decisions and resource coordination.
• Initiate discharge planning at admission and make referrals as needed.
• Provide education and support related to goals of care.
