Job summary

Kindly Direct Care is seeking a compassionate Registered Nurse for a flexible part-time RN Case Manager role serving individuals with developmental disabilities in Southern New Jersey. You will perform home-based assessments, create tailored care plans, and train Direct Support Professionals on best practices to support client independence and well-being.

You will conduct monthly check-ins, maintain meticulous patient records, and collaborate with families and care teams to ensure high-quality

Qualifications
