Beth Israel Deaconess Medical Center, located in Boston, MA, is seeking an RN Case Manager to drive patient transitions within the Triad Model of Care. You will partner with the interdisciplinary team to ensure timely discharges and efficient use of clinical resources, supporting high-quality care.

The role requires 3-5 years in acute care, MA RN licensure, and certifications such as ACM/CCM/CMAC (BLS required). This position offers opportunities to influence throughput and patient outcomes.

Qualificações

• RN licensure in MA (required).
• Bachelor’s degree in nursing or healthcare-related field preferred.
• 3-5 years in an acute care setting.
• ACM, CCM, or CMAC certifications preferred; BLS required.

Responsabilidades

• Reviews cases within 24–48 hours of admission/bed placement and throughout stay to plan discharge.
• Collaborates with medical team to formulate treatment plan including care transitions and patient flow.
• Conducts initial assessments to identify barriers to length of stay and discharge planning.
• Navigates care delivery system with physicians and departments to ensure timely tests, treatments, and procedures.
• Communicates the plan of care to the team and patient/caregiver; intervenes to maintain progression.
