Job summary

Beth Israel Deaconess Medical Center is seeking a Part-time RN Case Manager to support the Triad Model of Care Transitions. You will partner with an interdisciplinary team to facilitate patient flow and timely discharge, ensuring efficient use of clinical resources and high-quality care.

Responsibilities include conducting initial assessments, coordinating with social workers, and communicating the plan of care to patients and families.

Qualifications

• Massachusetts RN license is required.
• 3–5 years in an acute care setting.
• ACM, CCM, or CMAC preferred; BLS required.
• Strong communication and teamwork abilities.

Responsibilities

• Collaborates with care team to develop plan of care and patient flow.
• Reviews admissions within 24–48 hours to project LOS and discharge needs.
• Facilitates transitions and communicates discharge plans to patient and family.
• Documents care plans, barriers to discharge, and follow-up needs.
• Interacts with payers to verify coverage for discharge needs.
