Beth Israel Lahey Health is seeking an RN Case Manager for the Triad Model of Care Transitions to partner with the interdisciplinary care team and facilitate progression of care for hospitalized patients. You will drive throughput, ensure safe discharge, and navigate the health system to support effective transitions.

The role emphasizes collaboration, adherence to regulatory standards, and efficient patient flow with a focus on quality outcomes.

Qualificações

• RN licensure in the state of Massachusetts is required.
• Bachelor's degree in nursing or another healthcare-related field is preferred.
• 3–5 years of acute care experience is required or preferred.
• ACM, CCM, or CMAC certifications are preferred; BLS is required.

Responsabilidades

• Review cases within 24–48 hours to plan care progression and anticipated length of stay.
• Coordinate transition plan with social workers, direct care providers, and other hospital departments.
• Communicate the plan of care and discharge planning with patient and family/caregiver.
• Monitor length of stay, readmissions risk, and ensure timely follow-up with post-acute providers.

Formação académica

RN licensure in Massachusetts
