Job summary

Beth Israel Lahey Health is seeking an RN Case Manager to collaborate with the interdisciplinary team to drive efficient, high-quality patient care and timely discharges in the Triad Model of Care Transitions.

You will assess admissions, coordinate care plans, and ensure appropriate utilization of resources while maintaining compliance and patient satisfaction. Acute care experience and MA RN licensure are required; several certifications are preferred.

Qualifications

• Must be a licensed RN in Massachusetts.
• Bachelor’s degree in nursing or healthcare-related field preferred.
• 3–5 years of acute care or hospital experience.
• ACM, CCM, or CMAC certifications preferred.

Responsibilities

• Collaborate with the health care team to develop the plan of care and patient flow.
• Coordinate transitions and discharge planning with social workers, care providers, and post-acute partners.
• Identify barriers to length of stay and discharge planning; communicate with the team.
• Ensure timely tests, treatments, and procedures; advocate for appropriate care.
• Document care plans clearly and communicate progress to patient and family.

Education

RN licensure in Massachusetts

Bachelor’s degree in nursing or healthcare-related field
