Job summary

Christian Living Communities | Cappella Living Solutions in Colorado Denver is seeking a Care Transitions Manager RN to lead the care transitions for residents in our skilled nursing community and to partner with acute and post-acute providers to optimize outcomes.

Qualifications

• Registered Nurse in good standing.
• BSN preferred.
• 2+ years of nursing experience as RN or LPN.
• 1-3 years in a supervisory capacity.
• Knowledge of Colorado long-term care regulations.
• Experience with PPS, MDS, OBRA and OSHA preferred.
• Computer word processing and database experience.
• Excellent customer service; English fluency.

Responsibilities

• Coordinate the work of team members to meet resident goals.
• Perform post-admission screening to determine resident knowledge of disease processes.
• Assess resident activation level using PAM to coach improvements.
• Develop coaching relationships with residents and stakeholders and empower participation in care plans.
• Oversee discharge planning, care conferences, and case management.
• Implement INTERACT tools and contribute to quality improvement initiatives.
• Assist Director with supervising staff, training, and performance appraisals.
