Job summary

Christian Living Communities (CLC) | Cappella Living Solutions (CLS) are dedicated to transforming senior living with a focus on citizenship and diversity. The Care Transitions Manager RN will lead interdisciplinary care coordination to ensure smooth transitions from skilled nursing to home settings, guiding residents and families, and partnering with rehab services and vendors.

Responsibilities include supervising team members, coordinating care conferences, and promoting quality improvement

Qualifications

• Registered Nurse in good standing is required.
• Minimum 2 years of related nursing experience as an RN or LPN.
• 1-3 years in a supervisory capacity is required.
• Must understand state and federal laws for skilled nursing facility operation in Colorado.
• Proven leadership abilities and knowledge of healthcare transitions including PPS, MDS, OBRA and OSHA are preferred.

Responsibilities

• Coordinate team to meet resident goals and expectations.
• Perform post-admission screening to determine resident knowledge.
• Develop coaching relationships to empower residents and stakeholders.
• Follow up with residents, families and team to reduce readmission risk.
