Job summary

Christian Living Communities | Cappella Living Solutions’ Clermont Park Community is seeking an experienced RN with case management expertise to lead our rehab neighborhood and serve on the Community’s leadership team.

This role guides residents from skilled nursing to home, coordinates interdisciplinary care, supports families, mentors staff, and participates in on‑call rotations. Excellent communication, organizational skills, and vendor relationship abilities are essential to success.

Qualifications

• Active RN license in good standing
• Case management experience required
• Leadership or supervisory experience preferred
• Strong communication and relationship-building skills
• Comfortable managing vendor relationships and cross-team collaboration
• Willingness to participate in on-call rotation

Responsibilities

• Directs and coordinates resident care to align with goals and continuum-of-care
• Facilitates care transitions from skilled nursing to home with education for residents and families
• Supports residents in developing proficiency in managing their own care
• Fosters interdisciplinary communication across care settings
• Assists the Director in managing an assigned team and conducting performance appraisals
• Manages rehabilitation services, vendor relationships, and team competencies
• May act as Director of Nursing in the Director's absence
• Participates in on-call rotation
